Become a Member

To be come a chapter member, you must first be a Member or Candidate with ASHI®. If you are not a member or Candidate of ASHI®, visit www.ashi.org for more information. Becoming a chapter member is quite easy, simply print, and complete our chapter membership application and return it to us at your convenience. Our Membership roster is updated quarterly and we will add your information once all information is verified.
